CHECKLIST OF REQUIREMENTS FOR MEDICAL DEVICE & COSMETIC DISTRIBUTOR
- Accomplished Notarized Petition Form/Joint Affidavit of Undertaking
- List of Medical Device/Cosmetic Products to be importer/distributed
- Copies of Pharmacist Board Registration Certificate, PRC-ID, valid PTR, ID picture, Duties and Responsibilities, Certificate of Attendance of Owner/Pharmacist to a BFAD seminar on Licensing of Drug Establishments and Outlets
- Location Plan and Floor Plan (Office and Storage Room) with dimensions.
Photocopies of the following:
- If corporation, Registration Certificate with SEC and Articles of Incorporation or Partnership
- If single proprietorship, Certificate of Business Name Registration with Bureau of Trade Regulation
and Consumer Protection (BTRCP)
- Contract of Lease for the space of the office and storage to be occupied or any proof ownership if it is owned by the applicant.

If Importer:
- Foreign Agency Agreement from each supplier duly authenticated by the Territorial
Philippine Consulate
- Certificate of Registration of manufacturer and its conformity with GMP From Health
Authority/ISO Certification for medical device

If Wholesaler:
- A Valid contract with BFAD licensed supplier/manufacturer
- Certification that the product it sells are registered with BFAD
- LTO of manufacturer/supplier

If Exporter:
- A Valid contract with BFAD licensed supplier/manufacturer
- Certification that the product it sells are registered with BFAD
- LTO of manufacturer/supplier

Fees to be paid (Based on A.O. 50 s. 2001
- Medical Device Distributor - P 4,000.00
- Cosmetic Distributor - P 3,000.00

Must be presented upon inspection:
o Copies of R.A. 5921 and R.A. 3720 as amended
o Batch Distribution Record Book

2009 PPhA National Convention

Greetings!

It is with great pleasure that we announce that the 2009 Philippine Pharmacists Association, Inc. National Convention is set to take place at the Waterfront Cebu City Hotel on April 22-24, 2009.

This annual convention brings together the nation's top pharmaceutical practitioners and movers in the industry. This event provides the venue for setting the pace and mapping out new standards and directions that affect the industry and other stakeholders.

Aside from pharmacists, this convention is also of interest to trade associations, regulatory agencies, health professionals, drug organizations and other marketing and solution providers to the pharmaceutical industry.

The previous conventions have attracted an unprecedented number of pharmaceutical attendees. For the 2009 convention we hope to target at least 2,500 delegates and this can only be possible with your support.

We would appreciate it if you can encourage your members (of your respective areas) to come and join us for the 2009 Philippine Pharmacists Association, Inc. National Convention.

Your Cebu hosts are making sure that the convention will be educational, exciting and fun.

We will be sending the first waves of invitations to the PPhA members by early January. This will carry the final program and activities lined up for the convention.

GENERAL INSTRUCTIONS:

1. Your school/building assignment will be posted at the PRC premises one or two days before the examination. Bring your NOTICE OF ADMISSION when you verify your school/building assignment prior to the day of the examination.

2. Report to the school/building assignment before 6:30 a.m. on the first day of examination to verify your room and seat number. Be punctual, late examinees will not be admitted.

3. Examinees are required to wear their school uniform per Resolution No. 311 dated May 18, 1994.

4. Bring the following on examination day:

    1. Notice of Admission
    2. Official Receipt
    3. One piece metered-stamped window mailing envelope
    4. Two or more pencils (No. 2)
    5. Ballpens with BLACK ink only.
    6. One piece long brown envelope
    7. One piece long transparent/plastic envelope (for keeping your valuables

and other allowed items)

5. The following are PROHIBITED inside the examination premises/rooms.

    1. Books, notes, review materials and other printed materials containing coded data/information.
    2. PROGRAMMABLE CALCULATORS.
    3. CELLULAR PHONES, beepers, portable computers or similar gadgets/devices.
    4. Bags of any kind (ladies bag, shoulder bags, attaché case, backpacks, etc.
    5. Other examination aides not stated on this program.

NOTE: PRC WILL NOT BE ANSWERABLE FOR PROHIBITED ITEMS THAT WILL BE LOST.
